Charlos - this is a tough one - our district or our school psychologist usually supplied the forms or areas where they wanted data collected. If a student had visited a doctor outside of school then they usually provided the forms for the classroom teachers to collect data on the student.
Is your job to put the resources together so they have a variety to choose from for any given situation?
